You can search for a specific provider by last name, or refine your search based on the city, the zip code (3 or 5 digits), the state, area code, or a combination. If you can't locate your provider by name, please search by City and State. Then, click Find a Provider.

Our network is constantly growing and changing. If you cannot find your provider, or need help locating a provider near you, please contact us.

While we strive to provide the most up to date provider list for our members, it is possible that a provider may no longer participate in our network. We apologize for any inconvenience and encourage you to contact your selected provider prior to visiting their location.

Wal-Mart locations will not extend discounts on frames above the plan allowance, lens options, contacts lenses or contact lens fittings. Some providers may not offer discounts on prestige frames or amounts over the allowed benefit.
