Q: Where can I get a list of providers in my area?
A: Visit our website and enter your city and state for a complete list of providers in your area. You may also contact us at 1-888-729-5433 ext. 2013.

Q: Can I still visit any dentist I choose?
A: Yes, though the savings may be greater if you visit one of our participating doctors.

Q: What is the advantage to visiting a provider on the list?
A: Starmount has contracted with a number of doctors who have agreed to charge a lesser amount for services, which means less out of pocket expense for you.

Q: Where can I obtain a claim form?

Q: What is the mailing address for claims?
A: P.O. Box 80139, Baton Rouge, LA 70898-0139 (Dental), P.O. Box 14389, Baton Rouge, LA 70898-4389 (Vision).

Q: Can I check claim status?
A: Yes, our customer service department (1-888-729-5433 ext. 2013) will be glad to check claim status or check online. Click here to log in and access.

Q: How do I get another dental/vision card?
A: Contact us via e-mail (info@starmountlife.com) or call us at our toll free number (1-888-729-5433 ext 2013). Provide your name and member ID number and we will mail another card. If you have had a recent change of address, please provide that as well. Also order online through AlwaysAssist.

Q: Is my plan a policy year or calendar year?
A: This would depend on the particular plan - please check with your H.R. director or give us a call (1-888-729-5433 ext. 2013).

Q: What are the frequencies on procedures?
A: This will depend on your group's plan. Frequencies can be found in your policy or contact Customer Service.

Q: Can I get a breakdown of my benefits?
A: Yes. We can fax, mail or e-mail. Call (1-888-729-5433 ext. 2013) or e-mail us(info@starmountlife.com).

Q: When can I enroll in the vision/dental plan or add dependents?
A: This is usually done during your group's open enrollment period. Contact your human resource department for your open enrollment period.

Q: What are my co-pays on my vision plan?
A: Co-pays are listed in your policy or contact our Customer Service Department by phone (1-888-729-5433 ext. 2013) or email (info@starmountlife.com).

Q: Can I get contact lens and glasses?
A: You may elect contacts in lieu of frames and lenses. Please refer to your policy for frequency limitations.
