national network. We recognize benefit plan designs vary by group, industry and
region–which is why we also offer customized dental plans, features and options.
- Employee participation levels determine rates, not employer contribution
- Plans available for groups with as few as 2 enrolled employees.
- Dual-Option – Combine any two plans with as few as 10 enrolled (minimum
of 5 enrolled in each plan and subject to underwriting approval.)
- Takeover Credit available for employers and employees covered on the
prior plan at time of enrollment with comparable prior dental coverage.
- Freedom of Choice – Members may choose any licensed dental provider, or choose from our national network of participating dental providers.
